Transcript Requests
All requests for a copy of your official transcript must be in writing with your signature and the following:

• your name at the time of your enrollment • your social security number (student identification number)
• dates of attendance
• program of study
• address or addresses to which the copies are to be sent
• number of transcripts you are ordering
• a day time phone number and/or email address

Official transcripts are $3.00 for one and $1.00 for each additional in the same order.  If you would like your transcripts order to be mailed within 24 hours of receipt of your request and payment, there is an additional $10.00 charge . Also, please write RUSH in large letter on your request. For next day delivery (Federal Express), the cost is and additional $20 per address plus the $10 rush charge. If you have an account with an expedited delivery service, please included your account number with the $10 rush charge. Make check or money order payable to: Tulane School of Public Health and Tropical Medicine.

Graduation
Every student should apply for graduation at the beginning of his/her final semester. All degree candidates must wear academic regalia to the graduation ceremony. All encumbrances with a university office must be paid prior to graduation, or a student will not receive a diploma.

Adding/Dropping Courses
After registration, a student may add a scheduled course within two weeks after the beginning of a semester. A course may be dropped within the first two weeks of classes with no record appearing on a transcript. A student wishing to add or drop a course after the designated deadlines must have approval of the course instructor, advisor, departmental chair and the Dean for Admissions. A grade of “W” or “WF” will be recorded on the transcript after this point for courses dropped. A student incurs tuition and fee charges after the designated dates for dropping a course.


Withdrawing from School
Students who decide to withdraw from the School after the semester begins should discuss withdrawal plans with one of the deans and file a written petition to withdraw from all classes. The reason for withdrawal must be included in the written statement. A withdrawal for medical reasons requires an official letter from a physician. Any request for retroactive medical withdrawal cannot be approved once a student has completed his/her final examinations. Any student who leaves the School prior to the end of the semester without following proper withdrawal procedures risks serious impairment of scholastic records, loss of eligibility to continue at Tulane and loss of eligibility for complete or partial tuition refund.

Grievance Procedures
Cases of Alleged Discrimination Grievance procedures are available to all officially registered university students who allege discrimination based on race, sex, color, religion, national/ethnic origin, citizenship, marital status, sexual orientation, age, handicap, or veteran status. Any officially registered university student has the right to express a grievance without fear of loss of position or status. Any inquiries regarding possible discriminatory practices should be made to the affirmative action officer. Cases of Alleged Sexual Harassment of Students by Teachers; of Students by Peers Grievance procedures have been adopted by the university senate for all schools and colleges to address cases of alleged sexual harassment. Sexual harassment may include verbal or physical behavior. Students may address complaints of sexual harassment to the affirmative action officer or to the Dean for Student Affairs.

Honor Code
At every semester’s registration, each student pledges adherence to the rules and regulations of the school as stated in the code of Academic conduct. The code, which is under student self-governance, is aimed at eliminating dishonest practices in all academic endeavors. Any case of suspected infraction is investigated by the School Honor Board. Students are required to sign an Honor Pledge prior to participating in courses in the Tulane School of Public Health and Tropical Medicine. No Personal Identification Numbers (PIN) will be assigned until the Honor Code Pledge has been signed by the student and submitted to the Admissions Office.

Grading System
Instructors assign a letter grade reflecting the performance of each student in a course. Grade points are assigned to each letter grade based on a 4.0 point system and the number of credits for each course. Cumulative grade point averages are calculated each semester.

The “W” or “WF” may be administratively assigned for withdrawing from a course. “WF” (Withdraw with failing grade): the “WF” is assigned when a student withdraws from a course with a failing grade. Grades of “WF” are treated as “F” in computing cumulative grade point averages and remain on the record even if the course is later completed successfully. “W” (Withdraw without penalty): under special circumstances, a student is allowed to withdraw with the grade “W” if he/she has a passing grade at the time of withdrawal. The “W” does not affect the grade point average, but remains on the transcript even if the course is successfully completed.

Repeated Courses
In general, courses that have been completed with passing grades may not be repeated for the purpose of improving the grade. If a course in which an “F” or a “WF” was received, both the original and new grades will be recorded and counted in the cumulative grade average.

Right to Privacy
Privacy of students’ records and affairs is protected under the federal Family Educational Rights and Privacy Act of 1974 as amended (P.L. 93-380) and policies issued by the Tulane University Board of Administrators. Individuals who have particular questions are urged to contact the Office of Student Affairs.
